makes sense to divide 少阴兼表证 into two categories for treatment the cinnamon twig type person and the ephedra type person. for the former, we use the 太少 method as outlined above - first 四逆汤, then 桂枝汤. for the latter, no need for 先后 - either 麻黄细心附子汤 or 麻黄附子甘草汤.

note that with 苓桂术甘汤，we have a very similar situation:
“伤寒若吐若下后，心下逆满，气上冲胸，起则头眩，脉沉紧（hey isn't this almost like a 结胸脉?）。”

That's the problem with using vomiting or precipitation to treat a 伤寒. Well, what's worse is that sweating is used to treat this now...

“发汗则动经，身为振振摇者，苓桂术甘汤主之。”
A case of dehydration, a little like heat stroke.

Not too different than 真武汤's：
“...心下悸，目眩，身瞤动，振振欲擗地也...”
which is the direct result of over-perspiration - luckily he didn't try to 吐 or 下 first before inducing perspiration.

to say that 五苓散 is use to treat diarrhea is not the tightest link. it should be that 五苓散 is used to 利小便, and 利小便 can 实大便.

When exactly can 五苓散 be used to 利小便? most fitting is when there is dehydration, hence thirst. where dehydration somehow leads to 小便不利.
